You may leave the Health Mart franchise for any reason by giving us sixty (60) days advance notice and by following the steps outlined below. Note you must have satisfied all monetary obligations owed by you and your parent, subsidiaries and Affiliates to us and our parent,  subsidiaries and/or Affiliates.

To leave the Health Mart franchise: 

1. Inform your Pharmacy Sales Consultant (PSC) of your intent and email your sixty (60) days’ advance notice to HealthMartOperations@mckesson.com. 
2. You will receive a General Release Form via DocuSign to complete and submit no later than the first day of the month following your termination notice as to not incur any charges for the month following your termination.
3. Once signed and submitted, you will be sent further instructions regarding your duties and responsibilities, such as removing any use of Health Mart trademarks or wordmarks from your site and/or store.
