Checklist for Launching a Med Sync Program

Navigate Change to Pave Way for Success: Med sync can create opportunity to drive a pharmacy’s financial, operational and clinical success, but the commitment required from all staff members is high. Use this checklist to get your med sync service off on the right foot and set the stage for open communication for you and your staff to collaborate, course-correct and see med sync through to the point your pharmacy sees the benefits.

STEP 1 |

STEP 2 |
Recruit, enroll and manage patients (days 30–60). o Continue to target and recruit patients daily o Enroll patients as volume/workflow dictates (e.g., daily, weekly or bi-weekly) o Train additional staff members to ensure med sync bench strength o Maintain enrolled patients: - Check your daily tasks and identify patients who are ready to receive their 5-day call to: (1) review their med list; (2) confirm a refill pickup date o Hold staff meeting to review: - Med sync barriers; brainstorm solutions as a team - Milestones reached, celebrate success, set next short-term goals Note: Med sync adoption is challenging and it takes a while for a pharmacy to realize its benefits. Encourage staff to stick with it and to remember that enrolling patients is just the start — unless those patients are managed/synced each month, benefits will not be realized. |

STEP 3 |
Recruit, enroll and manage patients (days 60–90). o Continue to target and recruit patients daily o Enroll patients as volume/workflow dictates (e.g., daily, weekly or bi-weekly) o Maintain enrolled patients - Check your daily tasks and identify patients who are ready to receive their 5-day call to: (1) review their med list; (2) confirm a refill pickup date |

STEP 4 |
Drive pharmacy performance 90 days and beyond. o Enroll and maintain 30% of your patient population ages 50+ with 2 or more chronic medications o Maximize conversation at monthly pickup — engage patient in behavioral adherence coaching as well as other wellness services o Monitor outlier patients in EQuIPP and drive staff to improve patient PDC scores o Identify additional clinical services to offer patients to drive performance, e.g., adherence coaching, MTM services, immunizations, etc. o Continue to celebrate and encourage staff to drive pharmacy’s and patients’ performance leading to inclusion in high-performance networks, lower DIR fees and higher reimbursements |
