Franchise Termination

We value you as a Health Mart^® customer.

However, in the event you decide to no longer take advantage of the programs that Health Mart delivers, please be advised of the following:

1.	Terminate at any time for any reason after giving 90 days' prior notice.
2.	Bring account current with Health Mart and with McKesson Corporation through the effective termination date.
3.	Complete and return the Health Mart General Release Form. Once you notify your RSM of your desire to terminate, they will send via email a letter outlining your obligations upon termination and the General Release. Open the e-mail and click the orange View in DocuSign^® button. Follow the prompts to sign the General Release in DocuSign. Once you have successfully completed the General Release you will receive an email confirmation.
4.	Allow a Health Mart representative to enter the premises after the effective termination date so that we may verify the de-identification has been completed.
5.	Health Mart will disable your webpage (if applicable) after the close of business on the effective termination date.

Please refer to the Franchise Agreement which lists additional duties of the Franchisee upon termination of the Franchise Agreement.
