Prediabetes Risk Test

Do your patients know their risk for prediabetes and diabetes? Statistically, one in three adults and half of the elderly in your community have prediabetes and 90% of them do not know that they have it and are at risk for developing Type 2 diabetes.1 You can help them prevent diabetes by assessing their risk and recommending lifestyle changes.
HOW TO TEST FOR PREDIABETES RISK
- Spread the word about how your pharmacy is making a difference.
- Print and distribute the Prediabetes Fact Sheet for Patients to patients.
- Post the copy below on your pharmacy’s Facebook page:
- Are you at risk for Prediabetes? Stop by our pharmacy to take the Prediabetes Risk Test, and we’ll help you take control of your health.
- Download and use the accompanying social graphics.
- Print copies of CDC Prediabetes Risk Test (English and Spanish) and review the questions with your pharmacy team.
- Decide how your pharmacy will use the risk test:
- Distribute tests only:
- Distribute the Prediabetes Risk Tests to adults who visit your pharmacy. The test helps you assess your patient’s risk for prediabetes. For those who score 5 or higher, let them know about their risk and advise on how to make healthier lifestyle choices.
- Distribute tests and connect with providers: Distribute the Prediabetes Risk Tests to adults, counsel those who score 5 or higher, and refer them to providers (use this form) for testing and enrollment into the CDC-accredited National Diabetes Prevention Program.
