Health Insurance Claim Form (CMS 1500 Form) should be used to bill for both the vaccine product and the administration fee for each vaccine.  You may need to use a separate form to bill for each vaccine administered.  

You can pre-populate certain fields on the CMS-1500 form with appropriate ICD-10 codes and other information.
